2025 CHARGE CONFERENCES: Information & Instructions
Welcome to the Harpeth River District’s Charge Conference resource page. This is where you’ll find everything you need to complete your reports and stay on track during Charge Conference season.
OVERVIEW
All Charge Conference reports must be submitted online. No paper forms are needed.
The Charge Conference forms will be available at Mission Connect: Church Reporting. You will log in using your church number and the same password used for Ministry Financials or MC Stats.
All required forms must be uploaded at least two weeks before your Charge Conference.

GETTING STARTED
Once you are logged in:

Click on the Church Reports icon.

Open the Charge Conference Checklist.

Begin filling out each form listed in the checklist.
- Question in bold blue are required questions
Each required form will be listed with an upload option. Some are fillable PDFs; others are HTML forms completed directly in the browser. Some you will create a pdf of your own design to be uploaded.
Only the forms in the top section of the checklist are required.
The forms in the bottom section are optional and used at your church’s discretion.
INDIVIDUAL REPORTS
Required:
-
Charge Conference Minutes (new format: direct entry): Please complete through “Personnel” before your charge conference. The DS is your presiding elder.
- Attendance Record: The DS will bring a sign in sheet to your charge conference and the district office will scan and upload it afterwards.
- 2026 Clergy Compensation Form (downloadable Excel file): New format this year! On the “Full Comp to submit” tab, please fill out the blank fields, including SPRC Chair and Pastor signatures, print just that tab to a pdf and upload that pdf. Find more detailed instructions here. - Report of the Finance Committee (direct entry)
- Financial Policy (upload your pdf): if you answered “yes” to question 10.a. in the Report of the Finance Committee.
- Lay and Alternate Lay Member to 2026 Annual Conference (fillable pdf): Each charge may have one lay delegate per clergy in a charge.
- Report of the Pastor (direct entry)
- Report of the Trustees (direct entry)
- Environmental Impact Evaluation (upload your pdf): if you answered “yes” to question 8.a. in the Report of the Trustees.
- Environmental Impact Plan (upload your pdf): if you answered “yes” to question 8.b. in the Report of the Trustees.
- 2026 Committees of the Church (upload your pdf): Be sure to include at least the following:
- chair of the church council
- the committee on nominations and leadership development
- the committee on pastor-parish relations and its chair
- the chair and additional members of the committee on finance, the financial secretary, and church treasurer
- the board of trustees
- lay and alternate lay delegates to Annual Conference
- the recording secretary
- NEW! Safe Sanctuary Policy (upload your pdf at the “Additional Forms for upload” at the bottom of the check list.)
- NEW! Worship Times: please let us know your current regular weekly worp times
- NEW! Staff: The TWK Mission Spark Team is looking at programs to save local churches money on operating costs, particularly as they relate to staff and employees. Please indicate how many fulltime and parttime employees (not including the appointed pastor(s)) are at your church. Include any daycare/childcare employees.

Certified Candidacy Renewal (fillable pdf): For those who already are Certified Candidates. If you are ready to recomend a new candidate for ministry, that will go on your minutes
- Deacon Report (direct entry)
- Certified Lay Minister Report (direct entry)
- Certified Lay Servant Report (direct entry)
- Certified Lay Speaker Report (direct entry)
- Retired Clergy Report (direct entry)
- 2025 Wespath Contribution form (fillable pdf)
- 2026 Wespath Contribution form (fillable pdf)
- 2026 Youth Leader Contact Info (fillable pdf)
- 2026 Children Leader Contact Info (fillable pdf)
LEADERSHIP FORMS
There are two steps for reporting your church’s leadership for the upcoming year:
1. Upload the “2026 Committee of the Church” Report
Under the Charge Conference Checklist, upload a complete list of your church’s leadership for 2026. This should include all committee members, officers, and team leaders.
2. Complete the “Local Church Leadership” Section Online
This step updates the conference database and is required for certain leadership positions. Only required roles need to be entered here (they are marked with an asterisk in the system).
For each required position:

Include a start date.

Add an end date if the person is rotating off.

If the person is not listed in the system, click “Add New Person” and enter their contact information.
Note: The following roles must have a valid email address listed:

Lay Delegate to Annual Conference
Alternate Lay Delegate
